Referring your patients to the Get Healthy Service has numerous benefits, including:</h4><ul><li>Improved outcomes that complement patient care;</li><li>Visible contribution in reducing the health care burden in Australia;</li><li>Healthier community with better chronic disease management;</li><li>Focus on prevention &amp; early intervention;</li><li>Easy access &amp; referral systems in place; and</li><li>Effective evidence-based, high quality primary prevention.</li></ul><h3>WHAT THE SERVICE OFFERS</h3><h4>Participants receive:</h4><ul><li>Their own personal health coach</li><li>10 free coaching calls</li><li>Support to make changes over 6 months</li><li>An information booklet that provides additional information to support participants to achieve their goals</li><li>A coaching journal to record goals and actions</li><li>Access to a website where participants can track their goals and progress</li></ul><h3>PARTICIPANT ELIGIBILITY</h3><h4>To be eligible for the Service, the participant must be:</h4><ul><li>Over the age of 18 years and&nbsp;</li><li>Be a resident of New South Wales, the Australian Capital Territory, South Australia, Queensland or Tasmania.&nbsp;</li></ul><h4>To ensure that the participant is suitable for the ongoing coaching component of the Service, a number of screening and assessment questions are asked. This screening and assessment process asks about:</h4><ul><li>Any recent hospitalisations,&nbsp;</li><li>Chronic conditions,&nbsp;</li><li>Illnesses relating to the heart, lungs or brain,&nbsp;</li><li>Physical conditions, including pregnancy,</li><li>Mental health considerations and&nbsp;</li><li>Special dietary considerations.&nbsp;</li></ul><p>If these questions highlight any potential risks, the participant is referred to their General Practitioner to obtain medical clearance prior to taking part in the program.</p><h3>MEDICAL CLEARANCE &ndash; General practitioners</h3><h4>Some participants will require a medical clearance to participate in the Service. &nbsp;As a General Practitioner, you may be asked to determine whether your patient is:&nbsp;&nbsp;</h4><ul><li>Suitable for the Service.</li><li>Suitable for the Service with some considerations.&nbsp;</li><li>Not suitable for the Service.</li></ul><p>You are also encouraged to refer any patient you think would benefit from the <strong>Get Healthy Service</strong>. Make sure you eat according to your activity levels and watch your portion size. These include:</h4> <ul> 	<li>Significant decreases in weight&nbsp;</li> 	<li>Significant improvements in waist circumference&nbsp;</li> 	<li>Significant decreases in Body Mass Index</li> 	<li>Increased fruit consumption&nbsp;</li> </ul> <p><strong>Other findings:</strong></p> <ul> 	<li>Improvements in the proportion of participants undertaking recommended levels of physical activity from baseline to 12-months (increase of 5.2%), however the improvements made at end of the coaching program were not maintained at the 6-month follow up.</li> 	<li>Degree of improvement in vegetable consumption obtained at the end of the coaching program was not maintained.&nbsp;</li> 	<li>After adjusting for baseline levels and socio-demographic variables, the coaching program had significant maintenance effects for all anthropometric measurements and for fruit consumption.</li> </ul> <p>The figure below demonstrates the proportion of Get Healthy Service coaching participants who are classified as being a healthy weight and with &lsquo;no risk&rsquo; waist circumference after completing the coaching program and at 6-months post follow up (12 months from baseline).</p> <p><img height=""422"" src=""//gethealthy.siteflex.com.au/assets/images/baselineto12months_chart.jpg"" style=""margin: 0px;"" width=""642"" /></p> <h3>The Importance of Health Professional Referrals</h3> <h4>A study examining the profile of Get Healthy Service participants (who had completed the program)has shown some important differences in relation to the socio-demographic and risk factors based on their &ldquo;source of referral&rdquo;:</h4> <ul> 	<li>Participants referred by health professionals (including GPs) are more likely to complete the 6 month coaching program than those who are self-referred.</li> 	<li>A greater proportion of coaching participants referred by a GP was classified as obese (76.6%) and had a greatly increased waist circumference risk (88.6%) than those who are self-referred.&nbsp;</li> 	<li>78.7% of participants referred by a health professional were from the lowest two quintiles of advantage.&nbsp;</li> </ul> <p>These findings emphasise the important role that GPs and other health practitioners have in referring clients to the GHS.
